The Senior Transitions Coordinator plays a key role in supporting our Regional Directors and ensuring every client’s move into senior living is smooth, compassionate, and stress-free. This position is ideal for someone who thrives on organization, communication, and making a meaningful difference in the lives of seniors and their families.
In this role, you’ll oversee every step of the process — from scheduling tours and coordinating move-in logistics to maintaining relationships and following up after the transition. You’ll serve as a trusted guide, helping families navigate options, manage details, and feel supported every step of the way.
You’ll also help strengthen community partnerships, attend local events, and assist with marketing efforts that raise awareness about senior living resources. Success in this role comes from combining attention to detail with a genuine passion for service.
Senior care experience required.
The TCM Patient Navigator plays a vital role in helping seniors and their families navigate the transition from hospital to home. Through Transitional Care Management (TCM) and Social Determinants of Health (SDOH) resource coordination, this position helps ensure patients have the support, resources, and follow-up care needed for a safe and successful recovery.
In this role, you’ll collaborate with hospital case managers, healthcare providers, caregivers, and community partners to identify barriers to recovery, connect patients with community resources, and support continuity of care after discharge. You’ll build trusted relationships with patients and families while helping reduce avoidable hospital readmissions through compassionate, patient-centered care.
Success in this role comes from strong communication, sound judgment, and a passion for advocating for older adults during one of the most important transitions in their healthcare journey.
Senior care experience required. Experience in nursing, social work, care coordination, discharge planning, patient navigation, or other healthcare-related roles is highly valued.
As an Independent Contractor, you’ll have the flexibility to make your own schedule while helping seniors and their families navigate one of life’s most important transitions: finding the right senior living community to age with dignity.
In this role, you’ll work independently to guide clients through personalized care options, coordinate tours, and provide compassionate support every step of the way. You’ll collaborate with our network of senior living communities, hospitals, and care providers to ensure each client’s move is seamless and stress-free.
This position operates on a revenue share model, offering strong earning potential based on successful client placements and partnerships.
